For your safety and to ensure the best results, all clients are required to complete and sign a Eyelash Extensions Client Consent & History Form prior to their appointment.
Please complete the form and either:

This form includes important questions about your medical background, allergies, and the procedure, and must be submitted before we can begin any treatment.

*Fills appointments must be made within 2 to 3 weeks. Additional charges will apply if the appointment is made after 3 weeks. Appointment will be considered as a new set, once its 4 weeks.
*Quick touch ups appointment can be scheduled for emergency gap fill within the first 2 weeks if needed.
*Do not wear any mascara, contact lenses, and avoid coffee prior to your appointment.
*No phone calls, less talking and take a nap during service to speed up and best utilize your lash appointment time.
After Care:

Avoid getting your lashes wet for 24 hrs.

Do not rub or touch your eyes

Avoid exposure to excessive heat, sauna or steam shower

Sleep on your back

Do not apply any oil-based products around or on your eyes such as makeup, face cream or eye makeup remover

Do not use water-proof mascara. or eyeliner

Use gentle foaming facial cleanser to keep lashes clean

Clean and Brush your lash tips daily using a soft, bristle brush

Use oil-free eyelash-enhancing serum to minimize shedding

Avoid wearing contact lenses

Avoid spray or airbrush tanning
